Reporting a Claim or Incident

Because 5Star places coverage with multiple carrier partners for each Program, instructions vary for reporting a claim or a circumstance that may lead to a claim.  Specific claim reporting instructions are included with each policy we issue.  If the policy package can’t be located, please contact us by visiting our Directory for general contact information associated with each Program. 

All carriers stipulate that claims or incidents be reported as soon as possible.  Most offer 24 / 7  reporting options via a toll-free number , fax and/or email address

Typically, you or your client should be prepared to provide at least the following information:

  • Name of Insured
  • Address / Email Address
  • Policy Number
  • Name of Claimant (If other than Insured)
  • Date of Claim (Circumstance)
  • Description of Claim (Circumstance)
  • Additional information that may be pertinent to the claim or circumstance
