Program Parameters
  • No minimum production requirements
  • Producer appointments are not required at time of submissions, but they are required prior to release of quotes
Submission Requirements
    • ACORD 130 WC Application
    • Detailed Description of Operations
    • WC Supplemental Application (may be required based on industry and/or premium size)
    • Current Experience Modification Worksheet, if applicable/available
    • 3-5 Years currently valued loss runs within 90 days of effective date
